高線量率腔内照射が有効であった女性尿道悪性黒色腫

A case of woman with urethral malignant melanoma who got a better prognosis by high dose rate intracavitary irradiation

症例は83歳女性，尿道出血と会陰部腫瘤を主訴に2005年7月当科を受診。尿道から腟にかけて拇指頭大腫瘤が判明し，切除した。病理診断で悪性黒色腫と判明し断端陽性のため根治手術を予定したが，本人希望と年齢を考慮し放射線治療を選択した。経過は良好であったが，2010年5月CTで左外腸骨リンパ節に腫大が指摘された。しかし腫瘍による臨床症状はなく初診より5年間経過した現在，受診前と同じ生活の質を維持している。

A 83-year old woman was referred to our hospital with complains of urethral breeding and perineum mass in July 2005. A thumb-size urethral mass was removal and pathological findings revealed that the mass was malignant melanoma and previous excision was not complete. Considering her age and request,we selected radiation therapy instead of radical operation. Postradiation course was satisfactory. However in May 2010,follow-up CT detected a mass of 20 mm in diameter in her left inguinal lymph node,but,she had no symptoms so far. She has been well and keeping a good quality of life for five years.
