I．いとぐち

　著者らは本症について"ウィリス輪閉塞症：疾患単位としての病態と病像"と題して一文を草し，本症を独立した一疾患として取扱うべきことを提唱しておいた（1976）^10）．その根拠となったのは若年発病例についての追跡調査の成績である．それまでの年齢性別発病頻度統計をもととした推測に対して，改めて症例個々の経過の追跡を行って得た具体的資料に基づいてまとめたものである．しかし，当時，対象となりうる症例数が少なく，発病後の経過年数も短かったので，これを仮説として発表し，その末尾に「本症の完全な追究を行うには30年以上の歳月を必要とするため，推測に止めざるを得ない部分が残されている……」と記しておいた．その後の7年間に推測部分を明らかにしようと努力を払ってきたが，今回は，そのうち，出血発作の資料を手がかりに推察をすすめてみたいと思う．

This disorder, because of its protean manifestation,had long been controvertial until Kudo adovocated itas a distinctive disease appart from other apoplectiformdiseases. However, exact relationship between juvenileand adult types of this disease has been rather poorlyunderstood.

As the results of this study, authors concluded thatthis disease consists of two parts; initial and successive.Initial part corresponds to juvenile disease process andsuccessive to adult process. The former is the wellknown peculiar slowly progressive occlusive processwhich originates at the terminal portion of the bilateralinternal carotid arteries.
