外傷性急性大脳半球間裂血腫の1例

Acute Interhemispheric Subdural Hematoma: Report of a case

I．はじめに

　急性硬膜下血腫の多くは大脳円蓋部にみられる．動脈瘤や動静脈奇形のような脳血管病変を大脳半球裂に持つ例^1）を除き，一般の頭部外傷によって大きな血腫が，主としてこの大脳半球裂にみられるという例は極めて少ない．われわれはこのような，いわゆる外傷性急性大脳半球裂血腫の1例を経験したので，この例の検査所見およびその発現機序などにつき，若干の文献的考察を加え報告する．

A 46-year-old male was admitted to our service becauseof his head injury on May 20, 1980. He slipped about1 meter down into a brook himself during walk, and hithis head to the ground. Immediately after the accidenthe lost his consciousness about 30 minites and then hospitaliz-ed. About 6 hours later, level of his consciousness beganto go down gradually again with appearance of his left-sidedhemiplegic condition, which was more predominant in theleg than the arm.

Computed tomography demonstrated a large high densityin the inter-hemispheric space of his right side.
