I．はじめに

　大脳縦裂間硬膜下血腫は，Jacobsen^8）が報告して以来^{1,5,8,17,20）}，比較的稀なものとして報告されていた．

　これらの大多数は脳血管撮影の特徴的な所見から診断されていたが，最近ではCT scanにより，比較的容易に診断が可能となってきた．

The patient was 48-year-old male, who fell down on thesnow road and hit his occipital region. At the time ofadmission, he was in semicomatose state and right sidehemiparesis was noted associated with anisocoria.

The plain craniogram revealed diastasis of coronal suture.CT scan showed a high density with straight sagittal convexlateral border. Carotid angiogram demonstrated typicalseparation of the distal anterior cerebral arteries with con-vexity avascular area in right side.

This is an interesting report from both clinical and theradiological standpoint of a somewhat rare pathologicalentity associating with thrombocytopenia.
