急激な視力障害にて発症した海綿状血管腫の1例

A Case of Cavernous Hemangioma with Acute Visual Disturbance

I．はじめに

　硬膜や頭蓋骨より発生し，傍鞍部から中頭蓋窩を占拠する海綿状血管腫は稀な疾患であり，少数例の報告をみるにすぎない．また，従来より，臨床症状や神経放射線学的検査で特徴的所見に乏しいため，診断が困難なことが多く，治療上の問題も多いとされている．しかし最近，CT所見より術前に予測が可能であったとの記載も散見されるようになり^4,13,23），臨床像や治療方針にも新たな検討が加えられている．われわれは，急激な視力障害で発症し，極めて特異なCT像を呈した症例を経験した．本症における急性発症の機序とCT所見について若干の文献的考察を加えて報告する．

The authors report a case of parasellar cavernous heman-gioma having grown to the suprasellar region within 3 year'speriod and showing acute visual disturbance caused byintratumoral hemorrhage. A 44-year-old woman, who hadexperienced spontaneous intracerebral hematoma in theleft inferior frontal lobe 3 years ago and then obtained goodrecovery in the post-operative period had suddenly develop-ed decreased visual acuity of the left eye without otherneurological deficit.

CT scan revealed an irregular mass lesion in the chiasmalregion, which had a crown-like high density area over thesuprasellar low density.
