I．緒言

　転移性硬膜外腫瘍による脊髄圧迫はneurological emergencyの1つであり，放置すれば速やかに極めて悲惨な状態を惹起する．剖検によれば癌患者の約5％に発生していると報告されているが^3），近年悪性腫瘍に対する治療が進歩し患者の生存日数が延長するとともにこの神経系合併症の頻度は高くなる傾向にある．しかしこの疾患の治療法については議論が多く，いずれの方法によっても未だ満足すべき結果は得られていない^5,14,24）．その主な理由として臨床的研究が困難であり，また実験的にも今迄適当な実験モデルがなかったことなどにより系統的研究が少なく．この疾患の病態に不明な点が多いことがあげられる．

　最近Ushioら^21）はラットの第12または第13胸椎椎体前面に経皮的にWaker 256腫瘍を注入移植する方法で硬膜外腫瘍の実験モデルを作製することに成功した．彼らはそれを用い腫瘍による脊髄圧迫の病態の一部を明らかにし，さらにステロイド，化学療法，放射線療法，減圧椎弓切除術などの治療効果につき比較検討した^22）．しかしこの実験モデルは小動物であるラットを使用したため種々の操作，観察に制限があり，また実験腫瘍として使用したWalker 256 carcinosarcomaは，化学療法によく反応する腫瘍であり，必ずしも臨床にみられる各種の転移性腫瘍を代表していない．

We have developed an experimental model of epidural spinal cord compression in rabbits. VX₂ tumor cells injected anterior to the Th₁₃ vertebral body grow into the epidural space directly through the intervertebral foramina or distroying the vertebral bodies to compress the spinal cord. The animals developed weakness of the hindlimbs 2 to 3 weeks after inoculation and becamecompletely paraplegic within 2 weeks after the onset of weakness.

The histopathological examination revealed an invasion in the epidural space and vertebral body by the tumor.The compressed spinal cord showed edema and axonal swelling in the white matter.
