巨大脳動脈瘤の外科的治療

Treatment of Giant Aneurysms

Ⅰ．緒言

　1961年より1975年末迄に当教室で経験した1,080例のsaccular aneurysmのうち，手術所見または脳血管写上で最大径が2.5cm以上のものをgiant aneurysmとすると，その症例数は32（3.0％）である．一般にgiant aneurysmはその直接手術が困難でcarotid ligationが部位により適宜行なわれているのが現状であるが，当教室では積極的に直接手術を施行しその手術率は75％におよぶ．そこでこれら症例の症状，部位，手術法，手術成績および予後調査の結果等を呈示し，その治療法，手術適応につき検討報告する．

Thirty two patients with aneurysms measuring 2.5 cm or greater in diameter out of 1,080 patients with a saccular cerebral aneurysms were admitted to the Neurosurgical Department of Tohoku University School of Medicine.

There were 16 women and 16 men; 26 patients presented with subarachnoid hemorrhage and 21 patients with pressure symptoms and signs.
