Semeiology of myoclonus: History of confusion in classification

I．ミオクローヌスか，ミオクロニーか，ミオクローニアか―言葉の選択

　これから述べようとしている不随意運動を上のどの言葉で表現するのが適切か，という問題がある。西欧の諸国では後に述べるようにある一定の様式で使われているようであるが，我が国へは独，英，仏の各国からいろいろに用語が輸入され，しかもそれらの語尾が微妙に変化していることもあって，第1の混乱が言葉にみられる。そこでこのことから書き起こしたい。

　ミオクローヌス（マイオクローヌス）はMyoklonus（独），myoclonus（英）と書くが，独語でMyoclonusと書くこともある。ミオクローヌスはこの不随意運動そのものの名称であり，すなわち症状名である。これに対してミオクロニーはMyoklonie〔Myoclonie〕（独）と書き，またmyoclonies（仏）と書く。英語にはない。仏語は通常myocloniesと複数で書くが，時に単数でmyoclonieと書くこともある。この独語のMyoklonieは多くは症状名ではなく，そのような症状をもつ疾患，疾患群，疾患概念といったものを指している（例えば，Zur Genetik der essentiellen Myoklonie）。このMyoklonie（独）に相当するのがmyoclonia（英）である（例えば，classification of myoclonias）。

　Many authors have pointed out that formulating a definition of myoclonus is rather difficult and its symptomatology is complicated. The classifications of myoclonus and myoclonias are also widely varied and uncertain. All these facts are well known and indicate the confusion and controversy of the term myoclonus.

　First, in this paper, it is pointed out that the primary source of this confusion in Japan is due to terminology. Ill-digested terms originated from English (myoclonus, myoclonia), German (Myoklonus, Myoklonie) and French (myoclonies) have been misapplied and have caused further confusion.
