躁うつ病の治療

Treatment of manic depressive psychosis

　躁うつ病は，病相の起こり方や治り方に，特有な循環性，交代性，周期性などが見られる内因性の精神疾患である。また病相期を経過したのちにはほとんど欠陥を遺さないことにも大きな特徴がある。さらに精神分裂病より遺伝する率は高いことが知られている。このようなことから，躁うつ病は精神疾患のなかでも最も生物学的な病気であるとみなされてきており^25），おそらく脳内の代謝的な変動が関与した疾患であろうとの推定も行なわれてきた。

　本論文では，躁うつ病の治療について述べながら，その治癒機転を生化学的，薬理学的に考察することを試みた。とくに躁うつ病の病因に脳内monoamineが関与しているという仮説に対して，最近抗うつ剤の作用機序を根拠としていくつかの相反する学説が現われている点に注目して，それらの説を概観し，若干の見解をつけ加えた。

Abstraet

In spite of wide spread use of antidepressants, there are very few drugs to prevent the recur-rence of manic or depressive phase.

Three cases of bipolar patients whose phases were prophylactically treated were presented. The manic-depressive cycles of several years' duration of a 44-year-old man (Fig. 2a) was prevented by 200 mg of lithium carbonate with 2 mg of diazepam for these 10 years. A 71-year-old woman (Fig. 2 b) with a 33-year history of cycling between hypomania and depression recurring once a year was treated with carbamazepine after lithium was found to have side-effects on this patient.
