不眠症—ASDC, APSSによる新しい診断分類をめぐって

Insomnia: An overview of a new diagnosic classification of sleep and arousal disorders, proposed by ASDC and APSS

はじめに

　不眠症とは，夜間よく眠れないことを自ら悩む状態をいう。夜間の睡眠時間が6時間以下でも自ら全く苦痛を感じないで健康な社会的活動を維持している短時間睡眠者（short sleeper）と呼ばれる人びとは不眠症とはいわない。いっぽう躁病などの精神疾患のため夜間も休みなく動き廻り，ごく短時間の睡眠しかとらないにもかかわらず全く苦痛を感じない人びとは，通常の意味での不眠症にはあてはまらないが，平生睡眠を必要とする時間帯に客観的な不眠のあること，その結果周囲の人びとに苦痛を与えること，さらに夜間睡眠に質的異常の見られることなどから，不眠症の中に含めて取り上げることにする。

　不眠の訴えは日常診療の中で非常に頻度の高いもので，東大精神神経科で昭和45〜47年度の外来初診患者のうち約25％が不眠を訴えていた。

Abstract

　As a new classification of insomnia, the "Diagnostic Classification of Sleep and Arousal Disorders", proposed by Association of Sleep Disorders Centers (ASDC) and Association of Physiological Study of Sleep (APSS) in 1979 was explained indetails.

　The treatment of insomnia must be based on the correct diagnosis on the type of insomnias thus classified. Detection and removal of sleep disturbing conditions and psychotherapies are considered most fundamental. As an aid of treatment, sleep inducers and hypnotic agents now available in Japan were shown in Tables.
