I．緒　言

　大脳半球各部位の機能には，ある程度等価性があり，ある大きさ以上の病変がある場合，それがどの部位であっても痴呆を生じうる。脳梗塞について検討したTomlinsonの結果によれば梗塞巣の容積が50ml以上になると痴呆の頻度が有意に増加するという。いっぽう，それほど大きな病変でなくても，前頭葉，視床など特殊な部位の病変が知能に深刻な影響を与える場合もある^7）。いずれの場合でも，このような局所性病変においては病変と痴呆とを関連づけ，CTと痴呆を対比することは比較的容易である。

　むしろ問題が多いのは，より小さな散在性の病変と痴呆との関係である。このような病変は，痴呆を伴なう場合や痴呆を伴なわない場合がある。老年者で老人性変化を伴なう場合には，散在性の梗塞巣と老人性変化のいずれが痴呆の出現に関与しているか，という問題が生ずる。しかも散在性病変のCT診断は必らずしも容易ではない。

　Pathological findings of demented (89 cases)and non-demented, control subjects (74 cases)were compared in order to elucidate pathological basis of dementia in old people. Cases having large cerebrovascular lesions which were clearly the cause of dementia, were excluded from the study. The reliability of computed tomograms (CT) in the diagnosis was also studied. The results were as follows.

　1) Brain weight and the degree of ventricular dilatation were related to dementia, but the degree of convolutional atrophy showed no correlation with dementia.
