Dementia in normal pressure hydrocephalus

I．はじめに

　正常圧水頭症（normal pressure hydrocephalus，NPH）は，Adams，Hakimら^1,9）によって提唱された症候群である。この症候群は，1）成人で脳室が拡大しているのにもかかわらず髄液圧は正常範囲にあり，2）神経症状として，記憶力障害，自発性低下などの精神障害，歩行障害および尿失禁が見られ，3）シャント手術によって神経症状が改善する，とするものである。

　以後，同様な症例に対して世界各国でシャント手術が行なわれたが，精神症状，歩行障害，尿失禁の3症候が揃っていてもシャント手術が無効な症例や，臨床症候が揃っていなくともシャント手術が有効であった症例が認められるなど^{2,8,12,14,15）}，Adams，Hakimらが提唱したNPHの概念に多少混乱が生じてきた。

Abstract

　The mentation of 27 cases which reveal symptoms of dementia, gait disturbance and urinary incontinence in association with a dilated ventricle and normal cerebrospinal pressure was analyzed by a simplified mental examination consisting of items related to orientation, calculation, memory, general knowledge and idea concepts.

　These 27 cases were divided into two groups for the purpose of analysis. Group A consisted of cases which showed a improvement in the symptoms following a shunt operation. Group B consisted of cases in which the shunt operation was a failure.
