　Alexander病は中枢神経系に広範な髄鞘の形成不全を示すことから，白質ジストロフィに分類されており，通常は乳児に発症する。この乳児型の患者は生後1年以内に頭囲の拡大とけいれんや精神神経発達遅滞で発症し，多くは学童期以前に亡くなる。病理像が非常に特徴的でRosenthalfiberが柔膜下や血管周囲，とくに大脳白質に無数に出現する。Rosenthal flberはGFAPのほかに低分子量熱ショック蛋白質であるαB-crystallinやHSP27から成る凝集物で，ubiquitinによる修飾を受けている。Rosenthal flberがアストロサイトの細胞質内封入体であることから，以前からAlexander病はアストロサイトの代謝異常が原因だろうと推定されていた。最近，ヒトGFAP遺伝子を過剰発現させたマウスがAlexander病とほぼ同じ病変を生じたことから，Alexander病患者のGFAP遺伝子が調べられ，coding regionにde novoに点突然変異が生じていることが明らかとなった。

Alexander disease is a rare leukodystrophy that usually affects infants, and is characterized by the progressive failure of myelination in the CNS, and by massive accumulation of Rosenthal fibers in astrocytes. Rosenthal fibers are cytoplasmic inclusions that contain abnormal aggregations of GFAP, in association with the small heat-shock proteins, αB-crystallin and hsp27. Rosenthal fibers are also seen in other pathologic conditions such as chronic reactive gliosis and pilocytic astrocytoma, but are an especially prominent feature of Alexander disease.
