 Derangement of autoregulatory and chemical vasomotor responses is frequently encountered in ischemic brain. It has been postulated that their responses are controlled by neurogenic mechanisms, namely, by the autonomic nervous system, which is still in dispute with other hypotheses. We have been, clinically and experimentally, investigating the participation of the autonomic nervous system in cerebral vasomotor responses.

 Hemodynamic functional tests of the autonomic nervous system were performed with continuous recordings of blood pressure (BP), pulse rate and respiration. The parasympathetic nervous functions (reflex bradycardia of Aschner's eye ball pressure test and Valsalva maneuver) were supressed for a long period of time, but the sympathetic functions (cold pressor test and overshoot of Valsalva maneuver) remained normal or showed hypersensitivities except very severe cases with overt orthostatic hypotension in patients with acute cerebrovascular disease. Similar results were clinically obtained by the tests of pupillary dynamics and tear secretion. The parasympathetic functions were disturbed more easily and earlier than the sympathetic ones and orthostatic hypotension was not seen up to the end stage in experimental cerebral hypoxia or cerebral ischemia. The autonomic functions were damaged by the lesion of brainstem autonomic nuclei.
