昏睡時の瞳孔異常

はじめに

　昏睡に陥った患者を診察する際，臨床医にとって瞳孔検査は必須項目である。とくに神経学的検査ncurological examinationにおいて，瞳孔のチェックは最も重要な検査項目の一つに数えられている。これは瞳孔の大きさsize，形shape，位置positionを左右で比較し瞳孔反射を検査することによって，生命予後に大きな影響を与える脳幹部および自律神経系に対する情報が得られ，さらにこの情報が昏睡患者の予後と治療効果の判定に欠かすことができないという事実に基づく。

　本稿では，昏睡を痛覚刺激に対してのみ反応を示す以上の高度の意識障害と定義し，より軽度の意識障害と比較しながら若干自分達の成績も含めて瞳孔に関する知見を述べたい。

　The evaluation of pupillary functions is one of the most important parts of the neurological examinations. It has been well accepted that dilated and fixed pupils provide a grave sign in comatose patients with various kind of etiologies, thus, the analyses of pupillary size, shape and reflexes afford special oppotunities to decide the localization and severity of the damage in the nervous systems.

　In normal situations the pupils are round, equal and centered in the iris and any irregularity or abnormality in shape may indicate a significant meaning.
