レヴィ小体型認知症（DLB）は臨床・病理学的な症候群とされ，高齢者の認知症性疾患の原因として2番目に多い。病理学的に，レヴィ関連病理（Lewy bodies and neurites）に加え，アルツハイマー病病理変化（AD病理）である老人斑や神経原線維変化を認めることが多い。ここでは，DLBとAD病理との合併に関して述べた。近年の認知症の概念を理解するうえで，臨床医として，高齢者認知症患者における，脳病理変化が多岐にわたることを理解することは重要である。

Abstract

Dementia with Lewy bodies (DLB) is a clinically and neuropathologically defined syndrome and the second cause of dementia in elderly individuals. In addition to the presence of Lewy-related pathology (Lewy bodies and neurites), Alzheimer's disease(AD)-type neuropathologic changes such as senile plaques and neurofibrillary tangles are frequently present. In this review, the author focused on the co-occurrence of pathologic changes of DLB and AD. However, in order to understand recent concepts of dementia, clinicians need to realize that multiple brain pathologies are common in elderly patients.
