　58歳，男性．主訴は右側腹部痛，発熱，KUBにて右尿管末端に結石様陰影を，DIPにて右水腎水尿管を認めた．筋性防御出現し，虫垂炎の穿孔を疑い外科に紹介，開腹手術を施行した．限局性腹膜炎を伴う虫垂炎による水腎症であった。術後3週のDIPで右水腎水尿管は消失していた．

A 58-year-old man was admitted with right flank pain and fever. KUB showed a stone like shadow on the right side of the lower abdomen. DIP revealed hydronephrosis and hydroureter on the same side. Shortly after the admission, the muscular defence appeared. We suspected the perforation of the appendix and sent the patient to the department of surgery. Under the diagnosis of appendicitis, an emergency operation was done and an appendical abscess was confirmed. DIP which was performed 3 weeks later revealed normal urogram.
