恥骨後式前立腺全摘除術の手技上の工夫

RADICAL RETROPUBIC PROSTATECTOMY

　恥骨後式前立腺全摘除術の際の出血量の減少，術後尿失禁の防止に関して，二，三の工夫を行った。出血量の減少のためには，内腸骨動脈起始部にブルドック鉗子をかけ，また，陰茎根部をゴム紐で強く圧迫した。失禁防止の一助として前立腺の剥離を順行性に，前立腺と膀胱の間より始めた。両者の間を剥離するとき，膀胱頸部を保存するように行った。尿道と膀胱の吻合はVest法で行った。この方法で58〜79歳までの限局性と診断された前立腺癌患者6例の手術を行った。手術時間はリンパ節郭清，迅速病理を含めて平均4時間31分であった。出血量は平均620ml，輸血量は平均433mlであった。術後のコンチネンスは全例で保たれていた。

The technique of radical retropubic prostatectomy was modified to reduce the blood loss and to prevent the postoperative urinary incontinence. To lessen the loss of blood, bulldog clamps were applied at the origin of the internal iliac arteries and the base of the penis was fastened tightly with rubber tape. The procedure was commenced at the junction of the prostate and the bladder. The bladder neck was preserved to prevent postoperative urinary incontinence and to simplify the anastomosis of the urethra and bladder. This anastomosis was done according to the Vest procedure.
