精巣腫瘍の転移と誤認した後腹膜神経鞘腫

TESTICULAR TUMOR COMPLICATED BY RETROPERITONEAL BENIGN SCHWANOMA

　27歳，男性．右高位精巣摘除術を施行後，stage　ⅡA精巣未熟型奇形腫と診断した．初期治療として化学療法（PVB療法3コース）を施行したが後腹膜腫瘍の縮小がみられず，後腹膜腫瘍摘出術を施行した．病理組織学的診断はアントニーB型神経鞘腫であり，結果的には後腹膜良性神経鞘腫を合併したstage　Ⅰ精巣未熟型奇形腫であった．

A 27-year-old male patient was primarily diagnosed as stage II A testicular immature teratoma. In spite of three courses of PVB chemotherapy, retroperitoneal tumor had no change in size. Retroperitoneal tumor resection was performed and pathologic diagnosis was benign schwanoma (neurilemmoma). Final diagnosis was stage I testicular immature teratoma complicated by retroperitoneal benign schwanoma.
