　症例は56歳男性，主訴は左上腹部不快感。右腎は正常であったが，左腎は多発性の嚢胞を認めた。肝胆膵には異常は認めなかった。レノシンチで患側腎の機能低下を認め，腎摘出術を行い病理学的に成人型嚢胞腎と同一所見であるとの診断を得た。本例を片側性成人型嚢胞腎とするのか，またはまったく別の疾患であるのか結論はだせず，多発性腎嚢胞との鑑別について検討を加えた。本邦ではこのような症例は未報告で第1例目と考えられた。

A 56-year-old male was admitted with severe left abdominal discomfort. Family and past history revealed no renal disease. Body CT scan and RI renoscintirenogram showed multiple unilateral cystic lesion and decreased GFR in left kidney. The liver, pancreas and contralateral kidney were normal. Left nephrectomy was performed and the lesion was pathologically diagnosed as adult polycystic kidney disease (APCD). We discussed the differentiation of cystic diseases and concluded the diagnosis of unilateral APCD in this case.
