両側外鼠径ヘルニアを合併した成人型嚢胞腎

Adult-Type Polycystic Kidney Complicated with Bilateral Inguinal Hernias

　症例は32歳，男性。意識障害にて当院に入院した。クモ膜下出血の診断にて脳底動脈のクリッピング手術を施行した。入院中，下腹部腫瘤が認められ，精査・加療目的にて当科を紹介された。超音波，CT,MRIなどで成人型嚢胞腎および両外鼠径ヘルニアと診断した。腎嚢胞に悪性を疑わせる部位があり，加えて左の外鼠径ヘルニアが巨大であること，また嚢胞腎が大きいことから，悪性の否定と容量の減少を目的に嚢胞穿刺を施行した。その後，両側外鼠径ヘルニアの修復術は可能となった。ちなみに細胞診にて嚢胞液に悪性所見は認めなかった。

A 32-year-old man who underwent emergency clipping for subarachnoid hemorrhage was found to have an abdomi-nal mass and was referred to our department for thorough examination. CT scan and MRI identified adult-type poly-cystic kidney and bilateral inguinal hernias. The left inguinal hernia was too large to repair, so percutaneous cyst punc-tures were performed for mass reduction. Subsequent repair of the inguinal hernias was successful.
