嚢胞腎に合併した気腫性腎盂腎炎

A CASE OF EMPHYSEMATOUS PYELONEPHRITIS ASSOCIATED WITH ADULT POLYCYSTIC KIDNEY DISEASE (APKD)

　57歳，男性。発熱，左側腹部痛を主訴に当科を受診した。X線検査，　CTなどより嚢胞腎の所見と，腎実質内および腎周囲にガス像が認められ，嚢胞腎に合併した気腫性腎盂腎炎と診断した。糖尿病の合併はなく，全身管理および化学療法を実施したが，肺化膿症を併発し，死亡した。

A 57-year-old male who had a family history of APKD was admitted to our hospital complaining of high grade fever and left flank pain. Plain abdominal films and a CT scan showed gas formation in the renal parenchyma and its surroundings. A CT scan also demonstrated multiple cysts in the kidneys and the liver. He was in a state of renal failure and died after 19 days of conservative treatment.
