76歳男性。無症候性肉眼的血尿を主訴に受診。直腸診で両葉に硬結を認め，PSA 43.1ng/mlと高値であった。超音波と腹部CTで右腎腫瘍，膀胱腫瘍を認めた。TUR-Btと前立腺針生検を施行した後に，根治的右腎摘除術・右腎門部リンパ節生検術を施行した。病理組織学的検査の結果，膀胱癌，前立腺癌のリンパ節転移，腎癌の同時性三重癌と診断された。調べ得た限り本邦20例目であった。

A 76-year-old man with the complaint of asymptomatic gross hematuria,revealed bilateral prostatic induration by digital rectal examination with elevation of PSA level to 43.1 ng/ml. Ultrasound and abdominal CT revealed tumors within the right kidney and bladder. After TUR-Bt and prostate needle biopsy,we performed radical nephrectomy as well as lymph node excision biopsy from the right renal hilum. Histopathological results revealed cancer of the bladder,prostatic cancer with lymph node metastasis as well as renal cancer,leading to the diagnosis of simultaneous triple cancer. To our knowledge,this is the 20th case observed in Japan.
