スニチニブ治療中に肺門部リンパ節転移から気管支出血をきたした転移性腎細胞癌

Bronchopulmonary lymph node metastasis of renal cell carcinoma complicated with bronchial hemorrhage with sunitinib therapy：A case report

症例は47歳，男性。2002年に左腎細胞癌，多発肺転移の診断で根治的左腎摘除術を施行。術後にインターフェロン療法を施行したが，徐々に肺転移，リンパ節転移の増大を認め，スニチニブ治療を開始した。2コース施行後に肺門部リンパ節より気管支出血をきたし呼吸不全となった。

We herein report a case of a 47-year-old man of renal cell carcinoma which pulmonary metastasis,who developed bronchial hemorrhage and acute respiratory failure during sunitinib treatment. He underewent right nephrectomy and as an adjuvant treatment,interferon was administered,however,multiple lung metastasis and lymph node metastasis gradually increased. Therefore,IFN therapy was switched to sunitinib therapy. After second course of sunitinib therapy,bronchial hemorrhage occurred and then patient developed acute respiratory failure. Care should be taken with bronchial hemorrhage in sunitinib therapy.
