　本邦5例目の泌尿器原発三重複癌症例を報告する。患者は71歳，男性。喫煙歴30本／日×50年。第1癌は1979年に診断された膀胱移行上皮癌（G1,pTa）で，生涯悪性進展を示さなかった。第2癌は1995年のTUR-Bt時に偶発的に診断されたT2NOMO中分化型前立腺腺癌である。1年8か月の外来不参期間ののち，1998年12月，腰痛を主訴に来院した。精査にてT3N1M1（L4椎体および肺）左腎癌と診断された。患者は1999年6月，腎癌で死亡した。

We report the fifth case of triple primary urogenital cancer in Japan. The patient was a 71-year-old man who had smoked 30 cigarettes a day for 50 years. His first cancer was transitional cell carcinoma of the bladder (G1, pTa) diagnosed in 1979, which subsequently recurred 8 times but had not progressed in his lifetime. On the sixth TUR-Bt in 1995, adenocarcinoma of the prostate (Gleason score 6, T2NOMO) was incidentally found and was controlled with oral antiandrogens. He was lost from our follow-up from April 1997, to December 1998, when he presented with a complaint of low back pain.
