視力障害を伴わず眼球運動障害を呈した蝶形骨洞嚢胞の1症例

A Case of Sphenoid Sinus Mucocele Presenting with Disturbance of Ocular Movement but No Dysopia

　はじめに

　蝶形骨洞嚢胞はその解剖学的位置から，眼症状を引き起こすことはよく知られている。眼球運動障害も眼症状の1つであるが，その多くは視力障害とともに出現する¹）。今回われわれは，眼球運動障害を呈したにもかかわらず，視力低下の自覚を伴わなかった1症例を経験したが，このような臨床像を呈した蝶形骨洞嚢胞の報告はこれまで少ない。そこで，本稿ではその臨床経過および症状の発症機序について，文献的考察を加えて報告する。

It is generally known that sphenoid sinus mucocele is accompanied by ophthalmological symptoms. But there have been very few reports on sphenoid sinus mucocele presenting with distur-bance of ocular movement without dysopia.

A 25-year-old male with sphenoid sinus mucocele complained of headache, left deep orbital pain and disturbance of left ocular movement. Examination revealed left abducent nerve paralysis and oculomotor nerve paresis. After operation the symp-toms were improved. Six cases of posterior para-nasal sinus mucocele presenting with disturbance of ocular movement without dysopia including this case have been reported in Japan.
