I．緒言

　蝶形骨洞嚢胞はその解剖学的位置から視力障害をきたすことが多く，また嚢胞の開放により視力の回復が期待できる疾患^1）である。しかし全盲をはじめとする高度視力障害例の予後は不良とされ正常視力への回復は稀^1,2）である。今回われわれは失明後9日目に開放術を施行し著明な視力同復を呈した症例を経験したので報告する。

A 54-year-old male referred from Department of Neurosurgery with chief complaints of left blindness and headache is reported.

He visited our clinic on the 9th day after the onset of blindness. CT, MRI and other exami-nations revealed the existence of a cystic mass in the left sphenoid sinus. Immediate transnasal drainage restored almost complete visual acuity in a month.

It is stressed that both early diagnosis and im-mediate drainage are essential for the cystic lesions of the posterior paranasal sinuses with severe visual disturbance.
