眼瞼および眼球運動障害をきたした蝶形骨洞嚢胞症例

Pyocele of the Sphenoid Sinus Associated with Disturbance of Eye Movement and Ptosis

　I．はじめに

　蝶形骨洞mucoceleおよびpyoceleは比較的稀有な疾患とされている。最近，左上眼窩裂症候群を伴った左蝶形骨洞pyoceleの1例を経験し，幸い手術により劇的な症状の改善，消失をみたので報告する。

　A 37-year-old male, suffered from severe headache and double vision due to a pyocele of the sphenoid sinus is reported.

　X-ray examination and CT revealed a density in the sphenoid sinus which was surgically explored through the left maxillary sinusand ethmoid sinuses. Pyocele was confirmed and it contained ca 5ml of pus.

　Postoperative course was well, and ophthalmic complaints were completely disappeared in 4 weeks. Etiology, diagnosis and therapy of this case were discussed.
