はじめに
悪性腫瘍細胞の核DNA ploidy解析は,癌の診断にきわめて有用な腫瘍マーカーの1つであり,悪性度の判定や予後推定の指標として利用されている1)。さらにFlow cytometry (FCM)の使用により,大量の細胞を短時間に客観的に測定することが可能となり,各領域の悪性腫瘍の核DNA量と病期,予後などとの関連性を示す結果が集積されている2)。頭頸部悪性腫瘍にも核DNA量の異常が多くみられ,頭頸部扁平上皮癌進行例におけるDNA ploidyの解析はその悪性度の判定,予後予測のパラメータになるのではないかと考えられている。今回われわれは頭頸部扁平上皮癌進行例の手術凍結材料を用いploidyパターンと臨床,病理の因子との関連性を検討したので,その結果を報告する。
Cellular DNA contents of 72 advanced head and neck squamous cell carcinomas were examined. DNA aneuploidy was observed in 40 of 72 tumors (56%). The aneuploidy pattern was most frequent in hypopharyngeal carcinoma (83%), and laryn-geal carcinoma appeared to be the least one. There was no significant difference in DNA contents between the primary tumor and the metastatic lymph nodes. The survival rate was significantly lower in the cases with aneuploidy pattern than in those with diploid, especially in pharyngeal car-cinoma. Our results suggested that the DNA ploidy was the useful prognostic tool in head and neck carcinomas.
