　はじめに

　リンパ管腫はリンパ組織由来の先天性疾患であり，頭頸部領域に比較的頻度が高いとされている。この腫瘍は再発しやすく，また頭頸部領域には重要な脈管や神経が多く存在しているため，手術療法を選択する場合には術前にその性質と範囲を的確に把握することが重要である。近年のMRIなどの画像診断の進歩により，頭頸部領域のように解剖学的に複雑な部位でも術前診断が容易となりつつある。今回われわれは耳下腺深葉から副咽頭間隙へ進展したリンパ管腫症例を経験し，その治療法を選択するうえでMRIの画像診断が非常に有用であったので報告する。

A 2-year-old female infant presented with a cystic mass (40×35mm) of the right parotid por-tion. MRI showed that a multi-cystic tumor with different intensity occupied in the right parapharyn-geal space. The tumor was surgically removed by the transparotid and transcervical approach. At exploration, the tumor capsule was found to fuse with the deep lobe of the parotid gland where the tumor might have originated. The tumor was diagnosed as cystic hygroma by pathological studies. For 1 year after the operation, neither recur-rence nor cranial nerve palsy had been found. Our experience indicated that MRI was very useful for the diagnosis and treatment.
