　はじめに

　耳下腺および耳下部の嚢胞には，唾液腺導管由来の嚢胞，鰓原性嚢胞，類皮嚢胞や類表皮嚢胞（表皮嚢胞）などがある。多形性腺腫は唾液腺腫瘍の過半数を占める頻度の高い腫瘍であるが，その嚢胞形成についてはあまり知られていない^1,2）。今回われわれは，頬部の巨大な嚢胞形成を伴う多形性腺腫症例を経験したので，若干の考察を加え報告する。

A 87-year-old patient presented with pleomorphic adenoma forming a large cyst in the left parotid region. The tumor, 96×69×62mm, was removedsurgically. The cyst wall of the removed tumor was lined by simple squamous epithelium and filled with greenish fluid. Formation of large cyst in pleomor-phic adenoma is relatively rare. We surmised that the tumor was originated from an aberrant salivary gland and the cyst was formed with a normal sali-vary duct being obstructed by tumor itself or by trauma. Importance of differential diagnosis of cysts in neoplasms from simple cysts before surgery is stressed.
