頬部軟部好酸球肉芽腫症例

A Case of Eosinophilic Granuloma of the Soft Tissue

　はじめに

　軟部好酸球肉芽腫は，木村氏病^1）ともいわれている。原因不明の比較的稀な疾患で，全身の軟部組織，特に耳下腺部，顎下腺部，頸部，肘部などに単発性ないしは多発性に発生する。無痛性の軟部組織腫瘤あるいはリンパ節の腫脹を主訴として，血中の好酸球およびIgEの増多と，特異な病理組織像によって診断される。しかし初診時耳下腺腫脹を主訴とする場合が多く，耳下腺腫瘍との鑑別は必ずしも容易ではない。今回，われわれは耳前部，頬部から耳下部にかけて発生した巨大な軟部好酸球肉芽腫症例を経験し若干の知見を得たので報告する。

A case of eosinophilic granuloma of soft tissue (Kimura's disease) involving the parotid and the cheek region was reported. T₁ intensity MR ima-ging was useful for diagnosis of Kimura's disease. Blood examination showed a remarkable increase of eosinophiles and serum IgE. The allergic RAST test was found positive to Candida.

The surgical treatment and steroid treatment was mainly performed and tumor size of head and neck region was effectively reduced. Dissec-tion of the tumor with skin produced a favorable result cosmetically. The administration of steroids reduced the tumorous swelling, but after cessa-tion of them the swelling recurred gradually.
