　はじめに

　耳下腺腫瘍の外科手術では腫瘍を完全摘出することのみならず，顔面神経の保存に努めることが重要である。したがって術前画像診断の目的は第一に腫瘍の質的診断であり，第二に腫瘍と顔面神経との位置関係を明確にするための部位診断である。従来の画像診断法では直接に顔面神経を描出することは困難であるため，耳下腺周囲組織と腫瘍との位置関係より間接的に部位診断を行っていた^1〜4）。しかし近年，MRIの発達により，腺内顔面神経の直接描出が可能となり，部位診断における有用性が報告されている^5〜9）。顔面神経を直接描出することができればこれほど確実な部位診断はない。そこで今回，われわれは現在比較的普及している中磁場装置（0.5T）にて腺内顔面神経の描出を試みたので，耳下腺腫瘍におけるMRIの有用性について若干の考察を加えて報告する。

Using 0.5T-MRI, 12 cases with parotid tumor and 2 cases with no tumor were evaluated to identify the intraparotid facial nerve. On 8 tumor sides and 12 normal sides of these cases, the images were taken, using volume imaging with SPGR method and thereference line drawn between the inferior margin of the medulla of the atlas and the superior margin of the medulla of the mandibule. Facial nerve was identified in 7 of 8 tumor sides and 9 of 12 normal sides, and the findings were confirmed in 3 operated cases. On 7 cases (14 sides) using different condi-tions, facial nerve was not identified. The usefulness of MRI for preoperative examination of parotid tumor was discussed.
