　はじめに

　蝶形骨洞粘液嚢腫は，1889年スエーデンのBerg^1）が報告して以来，現在までその報告は200例程度^2）である。CTやMRIにより，診断が比較的容易になった現在においても，この嚢腫が蝶形骨洞内に限局している早期に診断がつくことは少ない。嚢腫内の粘液の貯溜量が増加して洞内を充填してくると，嚢腫は周囲の骨壁を圧迫，破壊し，洞外に進展する。前外方や上方への進展が多いため，蝶形骨洞粘液嚢腫の症状は，洞の解剖学的位置により，眼症状や頭痛が圧倒的に多い^3,4）。われわれは，蝶形骨洞を充満せずに下方の上咽頭に進展し，鼻閉を唯一の症状とした，蝶形骨洞原発と考えられた粘液嚢腫の1症例を経験したので，報告する。

A 67-year-old female patient with a sphenoid sinus mucocele was reported, and its origin and extent were discussed. The muocele was extended to the inferoposterior part of the sphenoid sinus and to the nasopharynx. Because of this specific exten-sion, the symptom was only nasal obstruction. The cyst wall was in contact only with the mucosa of the tegmen and posterior wall of the sphenoid sinus, and there was no obstruction of the ostium of the sphenoid sinus. We assumed that the cyst was a retention cyst formed from a goblet cell gland in the sphenoid sinus mucosa, and that there might have been a defect of the inferior wall of the sinus.
