Ｉ．はじめに
蝶形骨洞は副鼻腔のなかで最も後方に位置するものであり，下垂体，脳底部と骨1枚で隔てられているにすぎない。したがつてその疾部患はしばしば脳神経外科の対象となり，本邦でも植木1）によりその膿瘍治験例が報告されている。われわれは最近，トルコ鞍内硬膜外膿瘍のかたちをとつた蝶形骨洞粘液嚢腫の1例を経験したので報告する。
A case of an intrasellar and extradural abscess due to a large mucocele of the sphenoid sinus has been reported.
A woman aged 49 years, was admitted with the complaint of headache and diminished vision for 1 year and 5 months. It was believed that she had a sphenoid mucocele or a malignant tumor of the sphenoid sinus infiltrating the sella turcica. A craniotomy revealed an intrasellar and extradural abscess due to a large mucocele of the sphenoid sinus. The content of the mucocele was removed, then, the sphenoid sinus was opened through trans-maxillary approach. After the operation, her vision improved, but CSF rhinorrhea and pneumocephalus occured. These complications disappeared gradually and she accepts well to her home-work now.
A brief discussion of the signs and symptoms, the diagnosis, and the treatment of the condition is given.
