緒言

　わが国における頭頸部領域の悪性腫瘍のうち，鼻副鼻腔悪性腫瘍の発生頻度は約25％を占めるがそのほとんどは扁平上皮癌であり腺癌は少なく，特にColonic-type Adenocarcinomaは極めて稀である。

　われわれは，上顎洞原発の組織学的に大腸腺癌に類似した高分化型乳頭状腺癌を経験したので若干の文献的考察を加えて報告する。

An extremely rare case of colonic type adenocar-cinoma in the nasal cavity and pa ranasal sinus in a 48-year-old male with complaining of severe epitaxis and nasal obstruction was reported. The pathologic diagnosis of the tumor was a highly differentiated papillary adenocarcinoma with chara-cteristics of colonic type. There were neumerous goblet cells. The patient was treated with radical resection of the maxilla with orbital and ethmoid exenteration, irradiation and chemotherapy, in-cluding inua-arterial infusion with 5-FU.

A few cases of adenocarcinoma of this type have been reported and they were of highly aggressive and of poor prognosis. This patient showed no evidence of recurrence of the tumor for 7 months after the operation.
