高度視力障害をきたした脊索腫の1例

Skull Base Chordoma Presenting Severe Visual Impairment

緒言

　脊索腫（chordoma）は胎生期脊索の遺残組織から発生する稀な腫瘍である。斜台に発生する腫瘍のなかでは最も頻度が高く^1），しばしば両側視神経に障害を及ぼす。また頭蓋底脊索腫の約1／3は上咽頭へ進展することがあり^2），耳鼻咽喉科医にとって上咽頭腫瘍の鑑別診断のひとつとして考慮すべき疾患である。

　しかし発生部位の解剖学的位置のため腫瘍の全摘が困難で，予後不良の疾患である^1,3）。視神経の圧迫による高度視力障害をきたした場合，一般に視力予後も不良である^4〜6）。

A 40-year-old male showed severely impaired visual acuity (v.d.=0.01, v.s.=0), CT scan re-vealed the involvement of bilateral sphenoid sin-uses and clivus. Three weeks after the onset of visual impairment, the operation was done by combined approaches, of transmaxillary, transnasal and transseptal. These combined approaches afforded an improved surgical field and made the subtotal resection of chordoma possible. The visual acuity was soon improved (V.d.=1.2, v.s. =1.0). The postoperative irradiation was followed (60Gy). To the best of our knowledge, this degree of improvement has not previously reported.
