上顎洞結石の1症例

Report of Antral Rhinolith

I．はじめに

　鼻副鼻腔領域での結石の報告をみると，固有鼻腔に発生する鼻石の報告は比較的多く，それに対して上顎洞に発生する結石の報告は少ない。

　今回われわれは咽頭異常感を主訴に受診した47歳男性の上顎洞に多発した上顎洞結石を経験したのでここにその症例を報告するとともに，収録した文献により上顎洞結石と固有鼻腔結石との臨床統計について比較検討を行った。

　A 47-year-old male presented the closure of the left middle meatus, and a diffuse radiopacity accompanied with bone destruction of the superior and medial walls of the maxillary sinus in Waters view, and multiple metallic densities protruding from the the maxillary sinus to the nasal cavity in CT scan.

　Caldwell-Luc operation was done and multiple antral rhinoliths were obtained.

　Histopathological studies using both light microscopy and electronmicroscopy showed a lamellar structure with a nucleus. An organic chemical analysis revealed calcium phosphate (32%), magnesium phosphate (68%).
