はじめに

　画像検査などによる診断精度の向上と，的確な病態把握に準じた治療法の進歩が，癌の根治性を高め長寿に貢献するにしたがい，複数の癌治療を余儀なくされる多重癌の報告が増加している。特に，口腔・咽喉頭領域の悪性腫瘍は，喫煙や飲酒などが発癌の誘因となり，field cancerizationの概念から，しばしば同じ口腔咽喉頭領域，あるいは食道に同時性または異時性に重複癌が発見されやすい。一方で，同じ頭頸部領域であり同じ気道領域ともいえる鼻副鼻腔領域や聴器に重複癌が発生することは比較的稀である。

　今回，上顎癌治療後に口蓋扁桃・舌根に異時性の重複癌，さらに外耳道癌をきたした4重癌症例を経験したので報告する。

　We report a rare case of multiple metachronous cancers involving sinonasal and temporal cancers. The patient was a 66-year-old male. The patient underwent partial maxillectomy and postoperative radiotherapy for maxillary cancer (cT3N1M0). Chemoradiation therapy was performed for cancer of the palatine tonsil(cT4bN1M0) two years after the first therapy, and resection from the oral cavity was performed for the base of the tongue (cT2N0M0) 8 years after the first therapy. Squamous cell carcinoma in the right external auditory canal was detected 16 years after the initial therapy. We diagnosed the external auditory canal cancer (cT2N0M0) and performed lateral temporal bone resection.
