動眼神経単独麻痺をきたした蝶形骨洞囊胞の1例

A case of isolated oculomotor nerve palsy with sphenoid sinus cyst

はじめに

　副鼻腔囊胞のなかで蝶形骨洞囊胞は比較的稀な病態であるが，解剖学的特徴から視力障害などの視器症状を呈する頻度が高い。これら視器症状のうち，視神経症状を呈することなく，動眼神経麻痺のみを認める症例はきわめて稀である。今回われわれは，複視，眼瞼下垂を発症し，蝶形骨洞囊胞による動眼神経単独麻痺を呈し，囊胞開放術を行った症例を経験したので報告する。

　Sphenoid sinus cysts are relatively rare in the group of sinus cysts；however, they are characterized by eye function disorders because of anatomical positional relationships. We report a case of sphenoid sinus cyst.

　A 71-year-old woman presented to our hospital with a chief complaint of headache, left ptosis, and diplopia. Computed tomography and magnetic resonance imaging showed a sphenoid sinus cyst, and oculomotor nerve palsy caused by the cyst was detected. We opened an ethmoid sinus cyst using endoscopic sinus surgery（ESS）, and the headache disappeared the next day. Ptosis disappeared two months after the operation, and her oculomotor function recovered completely three months after the operation.
