悪性腫瘍との鑑別に難渋した外耳道耳垢腺腫の1例

A case of ceruminous adenoma difficult to distinguish from malignant tumor

はじめに

　外耳道腫瘍は良性悪性を問わず，臨床所見が類似していることが多く，正確な診断に至らない場合がある。外耳道腫瘍のうち耳垢腺由来の腫瘍も良性・悪性のものがあるが，術前に悪性腫瘍が疑われたものの，摘出病理検査結果が耳垢腺腫と診断された例や，術前に耳垢腺腫が疑われたものの最終病理結果が耳垢腺癌であった例も報告されており，やはり診断に難渋することが多い。今回，耳痛を伴った外耳道腫瘍に対して術前検査にて診断できず，摘出により耳垢腺腫と診断された症例を経験したので報告する。

　External auditory canal tumors are difficult to diagnose because of the various histological findings. Herein, we report a case of ceruminous adenoma in the external ear canal. A 76-year-old male was referred to our hospital with pain in the right ear and swelling of the right external auditory wall. Fine-needle aspiration cytology could not provide a correct diagnosis, and CT showed no temporal bone destruction. However, fluorodeoxyglucose positron emission tomography showed accumulation in the right external auditory canal. The tumor was completely resected using a posterior ear incision for diagnosis and treatment. The pathological diagnosis was a ceruminous adenoma.
