外傷性眼窩内血腫に対し緊急減圧術を施行して失明を回避した1例

A case avoiding blindness by performing emergency decompression for traumatic intraorbital hematoma

はじめに

　外傷により眼窩内に血腫を生じるとコンパートメント症候群を引き起こすことがあり，失明や複視の原因となりうる。失明の機序としては網脈中心動脈閉塞や視神経の圧迫，虚血性視神経障害などが考えられている^1）。一般的に受傷から2時間を経過すると受傷前の視力に回復することは困難とされているが^2），それ以上経過していても視力が改善する可能性がある。予後は時間経過ともに悪化するため，迅速な治療介入が求められる^3）。今回，外傷性眼窩内血腫に対して眼窩減圧術を実施し，失明を回避した症例を経験したので報告する。

　Intraorbital hematoma caused by trauma can induce orbital compartment syndrome, which can lead to blindness and oculomotor nerve palsy. It is crucial to quickly lower the intraocular pressure to avoid functional impairment, which requires canthotomy and medial orbital wall opening. The patient was a 68-year-old man who underwent canthotomy and medial orbital wall opening because his left eye was bruised and developed orbital compartment syndrome. Six months after the surgery, visual acuity and oculomotor nerve palsy improved. As in previous reports, rapid decompression was considered necessary.
