 Intraorbital hematoma caused by trauma can induce orbital compartment syndrome, which can lead to blindness and oculomotor nerve palsy. It is crucial to quickly lower the intraocular pressure to avoid functional impairment, which requires canthotomy and medial orbital wall opening. The patient was a 68-year-old man who underwent canthotomy and medial orbital wall opening because his left eye was bruised and developed orbital compartment syndrome. Six months after the surgery, visual acuity and oculomotor nerve palsy improved. As in previous reports, rapid decompression was considered necessary.
