舌潰瘍で発症したメトトレキサート関連リンパ増殖異常症の1例

A case of methotrexate-associated lymphoproliferative disorder with a tongue ulcer

はじめに

　メトトレキサート（MTX）は，高い有効率と骨破壊抑制効果をもち，低分子の疾患修飾性抗リウマチ薬として用いられる機会が多い^1）。その一方で，1991年にEllmanら^2），1993年にKamelら^3）がMTXの中止によって自然退縮を認めたreversible lymphomaの症例を報告して以来，MTXとリンパ増殖性疾患（lymphoproliferative disorder：LPD）の関連は注目を集め，MTX関連リンパ増殖性疾患（MTX-associated LPD：MTX-LPD）として分類されるようになった。その後，TNF阻害薬などでも同様の報告が挙がり，現在ではこれらをまとめて，その他の医原性免疫不全関連LPD（other iatrogenic immunodeficiency-associated LPD：IID-LPD）と定義され，わが国でもその報告は散見されるようになった^4）。今回われわれは，舌潰瘍として発症したMTX-LPDの1例を経験したため，若干の文献的考察を加えて報告する。

　We report a case of methotrexate-associated lymphoproliferative disorder（MTX-LPD）detected by an intractable tongue ulcer.

　The patient, a 77-year-old man with a history of rheumatoid arthritis who was taking methotrexate, presented to us with an ulcerative lesion on the left lingual border. We performed repeated biopsies, and the findings were suspicious for diffuse large B-cell lymphoma. Based on the history of taking methotrexate, MTX-LPD was diagnosed. After withdrawal of methotrexate, the ulcer improved and has not recurred in more than 4 months. When examining patients with oral ulcers who take methotrexate, it is necessary to keep the possibility of MTX-LPD in mind.
