小児の頰部に生じた結節性筋膜炎の1例

Nodular fasciitis of buccal region: a case report

はじめに

　結節性筋膜炎は皮下組織，主に深在性筋膜に孤立性の結節を生じる良性の線維性病変である。病理学的には線維芽細胞の反応性増殖であるが，急速な増大や周囲組織への浸潤を認めることがあり，時に肉腫などの悪性腫瘍との鑑別が必要となる。30〜40歳台に好発し，小児での報告は稀である。

　今回，われわれは小児の頰部に生じた結節性筋膜炎を経験したため，文献的考察を加えてその概要について報告する。

　A 6-year-old male patient with nodular fasciitis of buccal region was reported and complained a painless subcutaneous mass. A fine needle aspiration cytology could not confirm the diagnosis because of the insufficient specimen. The contrast-enhanced CT scan of the neck suggested an accessory parotid gland tumor. The tumor was surgically removed. The final diagnosis was nodular fasciitis. He was free from tumor for at least 2 years after the surgery. Head and neck are the areas of predilection for nodular fasciitis in children. We should consider a possible nodular fasciitis if a head and neck tumor in children is noticed.
