はじめに

　副鼻腔悪性腫瘍の原発部位は上顎洞が最多であり，蝶形骨洞はきわめて稀である^1）。また，小細胞癌は肺に好発する悪性腫瘍であり，頭頸部領域での発生は稀である^2）。今回，われわれは蝶形骨洞に発生した小細胞癌の1例を経験したので，若干の文献的考察を加えて報告する。

　We report a case of small-cell carcinoma that originated in the sphenoid sinus. A 58-year-old man was admitted to the department of neurosurgery for further examination of headache. He was referred to our department because CT images demonstrated a tumor lesion in the sphenoid sinus. The pathological diagnosis was small-cell carcinoma. The patient received two courses of cisplatin-etoposide therapy followed by radiotherapy. After the therapy, the tumor size was almost the same, but tumor necrosis was suspected. Despite undergoing 10 courses of amrubicin therapy, the tumor remained progressive and the patient died 20 months after the first visit.
