傍咽頭間隙から発生した小児の脱分化型脂肪肉腫の1例

A case of dedifferentiated liposarcoma in the parapharyngeal space

脂肪肉腫における脱分化の概念は1979年にEvansによって提唱されたものである^1）。従来からの脱分化型脂肪肉腫の診断では，高分化型脂肪肉腫もしくはその再発病変に非脂肪性肉腫が存在することが必要とされてきた。今回我々は，画像所見にて脂肪成分を認めず免疫組織化学染色にて診断に至った，小児の傍咽頭間隙発生の脱分化型脂肪肉腫の1例を経験したので，若干の文献的考察とともに報告する。

A 12-year-old man was referred to our hospital because of a painless mass in his left parapharyngeal space. The lesion showed hypointensity on T1-weighted images and heterogeneous hyperintensity on T2-weighted images without fat intensity. After a surgical resection, the lesion was revealed to show positive for CDK4 and MDM2 by immunohistopathological staining, indicating dedifferentiated liposarcoma. Although dedifferentiated liposarcomas without fat component diagnosed on MR imaging are very rare, CDK4 and MDM2 may be helpful in diagnosing such tumors.
