はじめに

　耳鼻咽喉科・頭頸部外科領域における異時性重複癌は20％程度と，実臨床においてよく遭遇する。しかし，頭頸部癌の重複癌患者は，初回治療の影響や2次癌に伴う栄養障害から低栄養であることが多く，再発転移に対する根治術を実施しようにも体力的に難しい症例が多い。今回われわれは周術期において適切なアセスメントを行い，免疫栄養療法を行うことで合併症なく治療完遂しえた再発舌根癌の1例を経験したので，若干の文献的考察を交えて報告する。

　Metaplastic duplicated cancers are found in patients about 20％ with head and neck cancer simultaneously or asynchronously. These patients are often suffering from malnutrition based on severe disease state or previous treatment effect. Therefore, nutritional improvement is key point to perform radical treatment for multiple cancer patients.

　In this paper, we reported an experience of the patient of asynchronous multiple cancer with recurrent carcinoma of the base of the tongue. Although he had severe undernutrition before surgery, he could undergo complete resection and reconstruction surgery with no complications by careful assessment and proper immuno-nutritional therapy during perioperative period.
