はじめに

　ワルチン腫瘍は耳下腺良性腫瘍のなかで多形腺腫に次いで頻度の高い腫瘍であり，中高年の男性に優位にみられ^1），喫煙との関連も示唆されている^2）。多形腺腫は長期間のあとに癌化することがあり，多形腺腫由来癌（carcinoma ex pleomorphic adenoma）と称される。一方，ワルチン腫瘍は一般に多形腺腫と異なり悪性転化はきわめて稀とされている^3,4）。ワルチン腫瘍の好酸性上皮性成分（オンコサイト）と非上皮成分（リンパ系組織）はいずれも悪性転化の可能性があるが，上皮成分の悪性転化は0.3％程度とされている^5）。癌腫成分は扁平上皮癌^6），粘表皮癌^7），腺癌，唾液腺導管癌^8）など，そのほかさまざまな癌腫成分が報告されている。今回，ワルチン腫瘍が悪性転化したと考えられる2症例について文献的考察を加え報告する。

　Warthin's tumor is the second most common benign parotid gland tumor. On the other hand, carcioma arising in Warthin's tumor is extremely rare. We presented two cases of a 65-year-old female with a left parotid gland mass and slight pain, and a 75-year-old male with a left parotid gland tumor. Histologically, the tumors of both cases consisted of Warthin's tumor with multilayered hyperplastic arrangements of oncocytes and focal squamous and goblet cell metaplasia. Transitional zone between benign and malignant component in two cases were evident. Both cases have shown no recurrence and been alive to the present.
