耳下腺ワルチン腫瘍54症例の臨床的検討

A clinical study of 54 cases with Warthin's tumor in the parotid tumor

I．はじめに

　ワルチン腫瘍（Warthin tumor）はpapillary cystadenoma lymphomatosumあるいはadenolymphomaとも呼ばれ，耳下腺腫瘍では多形腺腫に次いで頻度の高い良性腫瘍である^1）。多形腺腫とワルチン腫瘍で耳下腺良性腫瘍の約90％を占める^2,3）。これまで多形腺腫とワルチン腫瘍の比率は5：1といわれてきたが^2），最近の報告ではワルチン腫瘍は増加傾向にある^4～6）。耳下腺内には顔面神経が走行しているが，良性であるワルチン腫瘍の場合，顔面神経の確実な温存が望まれる。今回われわれは，最近当科で経験したワルチン腫瘍手術症例54例を対象に，術前診断および術後合併症，特に術後顔面神経麻痺に対する検討を行ったので報告する。

　We reported a clinical study of 54 patients（51 males and 3 females）with Warthin's tumor treated in our hospital from 1999 to 2005.

　The accuracy for diagnosis of Warthin's tumor with fine needle aspiration（FNA）and ^99mTc scintigraphy were 64.8％ and 82.9％.

　Temporary facial nerve palsy after the operation occurred in 11 patients. The period until recovering was 5.9 weeks on the average. A significant difference neither the age or the operation time or the tumor size was admitted in the facial nerve palsy. The surgery for tumors in deep lobe was suggested that it was necessary to be done carefully.
