耳下部皮膚に自潰したワルチン腫瘍の1例

A case of ulcerating Warthin's tumor

はじめに

　ワルチン腫瘍は耳下腺，特に下極付近に好発し多形腺腫に次いで頻度の高い良性腫瘍である。喫煙歴のある中高年男性に好発し，発育は緩徐で，悪性化もほとんどないことから経過観察されることも少なくない。しかし，時に急に増大する場合や顔面神経麻痺をきたす場合が報告されている。

　今回われわれは耳下部皮膚に自潰し，出血のコントロール目的のため手術を要した耳下腺ワルチン腫瘍の1例を経験したので，若干の文献的考察を加えて報告する。

　We reported a case of ulcerating Warthin's tumor in the parotid gland disrupted the overlying skin. An 80-year-old woman with an ulcerated parotid tumor was referred to our department during hospitalization for treatment of heart failure, renal disorder, and a burn on her lower leg. Warthin's tumor was diagnosed following a biopsy of the tumor, and a partial parotidectomy of the superficial lobe was performed. Although skin disruption of Warthin's tumor is very rare, it may be mistaken for a malignant tumor and may result in overtreatment, such as radical parotidectomy with sacrifice of the facial nerve.
